Information we collect 

 

Personal data 

We will collect information from you over the phone, via email, through our website contact us form and in person during therapy sessions.  Personal information is any detail which can identify an individual and may include for example your name, address, email address and phone number.  The Data Protection Act 1988 and General Data Protection Regulation are the governing laws that define how your personal information is processed. At Taskeen Therapy the lawful and proper treatment of your personal information is important to us. 

 

Personal data will also be collected as part of the contracting form and can include medical reports/letters, booking sessions, adding details to waiting lists and session notes. 


 

Special category information

You may share information with your therapist about your medical and personal history and are in control with how much or little you share. Special category data includes information about your mental or physical health, your racial or ethnic background, your sex life or sexuality, your political opinions, trade-union memberships, or your religious or philosophical beliefs.

 

By agreeing to the terms of our services and sending us information including special category information, you agree to us processing this information as is necessary for the services you are accessing. For example: emailing the information on an enquiry form to your therapist.

 

How we use the Information that you provide us 

The main purpose of collecting your personal information is in order to deliver the services you have chosen. Your therapist will rely on the personal information you provide to assess if therapy is suitable for you as part of the initial assessment. Once therapy is agreed, the personal information will help the therapist to decide which therapeutic approach is clinically appropriate. All our therapists are working in accordance with the legal and ethical framework of their governing body, and as such are contractually obligated to the security of your personal information. 

 

We will not sell your information to third parties. 

If you request a call back option on our website we will collect your name, address and phone number. 

Over the phone your allocated therapist will take your name, address, date of birth, a summary of your symptoms and previous therapy experience. 

During therapy sessions face-to-face or online your allocated therapist will confirm your personal details as per the assessment form and take your personal history, which may include special category information. This information is all gathered in order for the therapist to fulfill their contractual obligation towards you in the provision of your chosen therapy service. 

 

Where we store your personal information 

Taskeen Therapy will store your personal information electronically on devices that are password protected.  Personal information in files is further password protected and only accessible by your allocated Therapist. Client names and contact details are stored separately to other personal information (in an anonymised format). 


 

How long do we keep your personal information?

At Taskeen Therapy we will only keep your personal information for the minimum amount of time that is necessary. We work in accordance with the guidance of the Information Commissioners Office. We may hold your information for a minimum of 7 years after you have terminated using our services however this may be longer in the case of a claim or for insurance purposes. 


 

Do we share information with anyone else?

We may need to share your information with your GP or Health Provider in exceptional circumstances such as risk of serious harm or abuse to yourself or someone else

 

Under the GDPR you have the right to manage your own data and can request the following:

 

  • Ask us for a free copy of the information we hold about you 

  • Ask us to make changes to your personal information for example if your name/address have changed

  • Ask us to send your personal information to a third party 

  • Ask us what data processing of your personal information is taking place 

  • In certain situations ask us to delete the information we hold about you 

 

There may be situations where you ask us to stop processing your personal information and we cannot do so for example in order to comply with the law or a regulation within the UK. We will always keep you informed if we cannot comply with your request. 


 

Use of Testimonials 

We share Taskeen Therapy client testimonials in order for real lived experiences to be accessible to users of our services. The testimonials are individual experiences and results do vary. Taskeen Therapy are not responsible for any of the client comments or opinions shared on the website. At Taskeen Therapy we do not claim that testimonials are proof of expected results, as every individual will respond to therapy differently. Testimonials do not make claim that our services can be used to diagnose, treat or prevent any disease.
